Just a reminder of the Enhanced Proof of Vaccination Requirements coming into effect this๐“๐ฎ๐ž๐ฌ๐๐š๐ฒ, ๐‰๐š๐ง๐ฎ๐š๐ซ๐ฒ ๐Ÿ’, ๐Ÿ๐ŸŽ๐Ÿ๐Ÿ ๐š๐ญ ๐š๐ฅ๐ฅ ๐‚๐ข๐ญ๐ฒ ๐จ๐Ÿ ๐’๐š๐ซ๐ง๐ข๐š ๐€๐ซ๐ž๐ง๐š๐ฌ , as per the Provincial requirements.
Please ensure all of your players, participants, coaches, team staff, visiting teams, officials, families and spectators have received the below/attached details and are aware that as of Tuesday, January 4, 2022 they will be required to show their Enhanced QR Code (on phone or the printed paper), and that no other forms of proof will be accepted.
Identification is still a requirement to accompany the QR code. And that effective January 10, 2022 for those who need to show proof of exemption this must also be shown only by their enhanced QR code as well.
How to Receive the Enhanced Certificate:
Individuals can download their enhanced certificate with QR code by visiting:
or by visiting local libraries or calling the Provincial Vaccine Contact Centre at 1-833-943-3900 to have their vaccine certificate mailed to them. They can also visit a Service Ontario centre to receive a printed copy.
Identification will still be required in addition to the enhanced vaccine certificate with QR code.
